You can ask for a repeat prescription in the following ways (click on each tab for more information)

Prescriptions via the NHS App

The NHS App gives you a simple and secure way to access a range of NHS services.

To access GP services online via the App, you must:

  • be registered with a GP practice in Wales that is connected to the App
  • have a fully verified NHS login, or a valid photo ID to set one up
  • be aged 16 or over

Local Pharmacies

The local community pharmacies are happy to manage your repeat prescription ordering for you.  If you wish your prescription to go to a nominated pharmacy please complete this nominated pharmacy form and return to the reception.

At the Surgery

Between 8.30am and 6pm you can drop off your repeat prescription form in the black post box in the surgery foyer at the main entrance to the surgery.

By Post

Prescriptions can be ordered by post.  If you wish for the prescription to be returned by post then please enclose a stamped self-addressed envelope.

**Please remember to order your repeat prescription 48 hours before you run out of medication.** 

If you don’t have your re-order prescription slip then please complete and return this form by email, by hand or by post. This form can also be found in the foyer of the surgery.

For all non-repeat medication you can complete a non-repeat medication request form, found here or in the foyer of the surgery. Please return by email, post or by placing in the black post box in the surgery foyer.
