You can ask for a repeat prescription in the following ways (click on each tab for more information)

My Health Online (MHOL)

If you register for a My Health Online (MHOL) account you can order your repeat prescriptions online. Please find the forms to request a My Health Online account below; alternatively please contact the surgery: MHOL registration form (Eng) | MHOL registration form (Welsh)

You can also register for My Health Online on behalf of another individual (for example, a parent or legal guardian of a child; to authorise someone else to use My Health Online on your behalf; or carers to those people who do not have the mental capacity to act in their own right): >> MHOL registration form (child or carer access)

Two forms of identification will be required on receipt of the completed form. Following this your account can be activated. Step-by-step instructions can be found here.

Local Pharmacies

The local community pharmacies are happy to manage your repeat prescription ordering for you.  If you wish your prescription to go to a nominated pharmacy please complete this nominated pharmacy form and return to the reception.

At the Surgery

Between 8.15am and 6pm you can drop off your repeat prescription form in the black post box in the surgery foyer at the main entrance to the surgery.

By Post

Prescriptions can be ordered by post.  If you wish for the prescription to be returned by post then please enclose a stamped self-addressed envelope.

**Please remember to order your repeat prescription 48 hours before you run out of medication.** 

If you don’t have your re-order prescription slip then please complete and return this form by email, by hand or by post. This form can also be found in the foyer of the surgery.

For all non-repeat medication you can complete a non-repeat medication request form, found here or in the foyer of the surgery. Please return by email, post or by placing in the black post box in the surgery foyer.
